What it involves
One hour in an older person’s home: reviewing all the medication they keep, spotting duplicates and interactions, and assessing fall risk by actually looking at the floors, the lighting and the bathroom.

After the visit
The family gets a report with the cleaned-up medication list and proposed changes to the home. This cannot be done over the phone.
